Extended Learning Course Registration Form 2010-2011
These forms are to be used by students to register for Distance Education and Intensive course offerings.
Registration Revision Form
Students can revise their course registration by using this form to drop or add a course during the registration revision period, to request a Voluntary Withdrawal from a course after the voluntary withdrawal deadline, request a Withdrawal (only for unusual circumstances with the approval of the Academic Dean).
Request for Incomplete Grade/Extension Form
Students unable to complete the term work prescribed for a course must apply to the instructor for approval, within two weeks prior to the end of classes, for an incomplete grade and time extension for completion of work. A grade of Incomplete may be granted by the instructor only when a major portion of the course work has been completed satisfactorily, but some requirement(s) have not been completed.
Request for a Letter of Permission
If you cannot find a Booth College course to fit your program of studies, you may be eligible to take a course at another educational institution by Letter of Permission. This is a request form only and does not imply permission to take the course(s) listed. A copy of the course description(s) indicating content and credit hours must accompany this request form.
Transcript Request Form
If you wish an Official Transcript sent directly to another educational institution or employer, please provide ample notice to our office for preparing the transcript. Initial transcripts are free but a fee is charged for subsequent transcripts. Copies of unofficial transcripts can also be requested at no charge.
Change of Name Form
If you have changed your name and wish to have this changed on your student record, please use this form. Official documentation of your change of name (eg: copy of marriage certificate) must be presented.
Change of Address Form
To ensure that your records are up-to-date, please inform Booth College of your change of address.

Graduation Application Form
Booth College Graduation occurs in April and October of each year. Students who have completed all certificate or degree requirements are eligible to apply for graduation.
